Repeat prescriptions can be requested in writing by post, email or via our website online fom. We do not accept requests by phone. 

Please allow 48 hours (excluding Saturday/Sunday and bank holidays) for preparation and signature.

You must be signed up with one of the pharmacies listed below if you wish to collect your prescription directly from them rather than from the surgery.   Collection services are offered by:

Lloyds Pharmacy Fair Oak, The Village Pharmacy Fair Oak, Colden Common Pharmacy.

A repeat request form attached to all computer printed prescriptions for your convenience. Requests made via the online form will be saved for you automatically.

Please request only items that you require to reduce waste. Do your medications run out at different times? If so, our pharmaceutical adviser may be able to to simplify your requirements, please contact her via the surgery telephone number.
